What are the responsibilities and job description for the Wound Therapy Associate - LPN position at UR Medicine Home Care?
Summary: Under the direction of the WOCN and/or wound clinic/provider the Wound Treatment Associate is responsible for provision of routine skin care and assisting with the provision of comprehensive wound care and monitoring for patients with skin tears, incontinence-associated dermatitis, dehisced incisions, pressure injuries, burns, lower extremity ulcers, diabetic foot ulcers and all other applicable wounds.
Schedule: Monday - Friday, Days 8 to 430
Qualifications
- Education:
- A graduate of an approved school of professional nursing, who is a Licensed New York State Licensed Practical Nurse (LPN).
- Satisfactory completion of the WOCN Society’s Wound Treatment Associate (WTA) Education Program preferred; however, willingness to gain the certification within a year is a requirement.
- Experience:
- Two (2) or more years of nursing experience, home care experience preferred.
Licensure, Certification, and/or Credential Requirements
- Currently licensed as a Licensed Practical Nurse in the state of New York
- Valid NY State Driver’s License and reliable transportation.
- Current BLS CPR certification or ability to obtain upon hire.
Responsibilities
Qualified individuals must be capable of performing the following essential duties and responsibilities, with or without reasonable accommodations:
- Collaborate with team members to provide routine care to maintain skin health.
- Identifies patients at risk for skin tears and/or incontinence associated dermatitis and initiates appropriate preventative care.
- Identifies and addresses causative and contributing factors to skin breakdown.
- Provides ongoing documentation of wound status and progress in wound healing.
- Selects appropriate dressings and maintains a physiologic environment for wound healing.
- Provides or assists with appropriate education regarding skin and wound care to patients, families, and staff.
- Participates in Quality Improvement audits and initiatives.

- E-Record (electronic medical record systems) EPIC experience preferred

Hourly Range: $24.14 - $33.31
