Summary:
The Intake Manager oversees and coordinates the intake of patients referred to home health services. This role ensures that all admissions meet clinical, regulatory, and payer requirements, and that referrals are processed efficiently to support timely initiation of care. The Intake Manager leads the team responsible for processing referrals, verifying insurance, obtaining authorizations, and coordinating communication among referral sources, clinical teams, and patients.
Schedule: Monday- Friday, Days 8am to 430PM
Minimum Qualifications:
- Bachelor’s (or higher) degree in nursing, Healthcare Administration, or related field (RN required)
- Experience with NYS CHHA regulations and reimbursement authorizations required
- Familiarity CMS Conditions of Participation required
- At least 1 year of supervisory or team lead experience in a home health setting required
- NYS Licensure as a Registered Nurse required
Responsiblities:
- Oversee the daily operations of the intake department, including referral processing, data entry, and coordination of home health admissions
- Ensure compliance with state and federal certified home health regulations, including Medicare Conditions of Participation.
- Maintain HIPAA compliance and ensure all patient information is handled confidentially
- Develop and implement procedures to improve intake workflows and reduce delays.
- Monitor key performance indicators (KPIs) such as time to admission and referral accuracy
- Accountable for supervision: of all Intake staff (HCC RN, HCC LPN, and Patient Service Representatives) 7 days a week.
- Responsible for performance management of Intake team, provides coaching, mentoring, and performance improvement plans as needed. Completes annual performance reviews.
- Manages staff work schedules, productivity, time off ensuring full daily Intake team coverage
- Provides leadership to ensure appropriate multidisciplinary communication, collaborative work relationships, and a customer-oriented environment responsive to the needs of all external/internal customers. Examples include QAPI meetings, Monthly leadership meetings, Monthly meetings with staff, working with Operation Excellence team on Intake PIP, meets monthly with hospital leadership.
- Establishes and ensures a safe work environment for staff to prevent injuries from occurring or recurring; examples include working with HR for special accommodation like elevated desks, ergonomic chair, work from home.
- Works closely with marketing, clinical team managers, and scheduling team, to support census growth.
- Other duties as assigned.
