About The Role
Provide trauma-informed, strengths-based case management and housing navigation to families experiencing homelessness. Manage a caseload of 21 households and support each to secure permanent housing within 120 days of program entry.

What You’ll Do
- Create individualized housing plans; identify and reduce barriers to stability.
- Ensure clients are document-ready (ID, SSN, income, vital records).
- Lead housing search: landlord outreach, applications, and subsidy navigation.
- Maintain accurate, timely HMIS data; meet all contract/agency standards.
- Conduct regular check-ins, assessments, case notes, and progress updates.
- Coordinate referrals (mental health, employment, childcare, benefits).
- Provide crisis intervention and conflict resolution as needed.
- Collaborate in team meetings, case conferences, and trainings.
- 2 years in housing/homeless services; interim/residential experience preferred.
- Working knowledge of housing programs, landlord engagement, and rapid rehousing.
- Proficiency with HMIS data entry/reporting.
- Commitment to trauma-informed, client-centered practice with diverse populations.
- Strong organization, documentation, and communication skills.
- Valid driver’s license, reliable vehicle, and current insurance/registration.
- Proven ability to work independently and thrive in a collaborative team.
