Job Summary
The Glycemic Advanced Practice Provider will serve as a full time APP to provide care to patients at SUNY Upstate Downtown and Community Campus. The APP will provide Inpatient Glycemic Management of the Diabetic patient.
Key Responsibilities Include
Minimum Qualifications
Master’s Degree as a Nurse Practitioner or Physician Assistant (or graduated from an accredited PA program).
NYS Licensed/Registered NP or PA.
Current National Certification consistent to the clinical practice.
NYS licensed/registered NP or PA required. Have a NPI number and be Medicare enrolled with a Medicare Identification Number (or PTAN) (or eligible for Medicare Enrollment within 6 months of hire.
CPR-BLS Certification required at the time of appointment.
Must successfully pass pre-employment and credentialing clearances.
Preferred Qualifications
Endocrine or Glycemic experience preferred.
Work Days
Monday - Friday, Days with possible weekend rotation.

- Providing expert level clinical care & coordination of care via a full range of services that will improve quality of patient care.
- Assessing & evaluating patients.
- Ordering appropriate laboratory testing & diagnostic studies.
- Arranging for referrals, testing, consultation & therapeutic services.
- Maintaining outstanding relationships with families, referring physicians & all members of the health care team.
