Join our team as a Behavioral Health Provider and make a meaningful impact by delivering integrated, person-centered, and recovery-oriented behavioral health services within the Certified Community Behavioral Health Clinic (CCBHC) model. You’ll provide comprehensive mental health and substance use assessments, treatment planning, psychotherapy, crisis intervention, and care coordination for individuals with co-occurring disorders—all while collaborating with a multidisciplinary Integrated Care Team.

Full-Time and Part-Time Opportunities available - This is a primarily on-site position at one or more of our Utica, Rome, and/or Herkimer locations. Hybrid work (1-2 days per week) may be approved based on role expectations, performance, and program needs. The final work location arrangement will be determined collaboratively during the hiring process.

Position Summary

The Behavioral Health Professional delivers integrated, person-centered, and recovery-oriented behavioral health services within the CCBHC model. This position provides comprehensive mental health and substance use assessment, treatment planning, psychotherapy, crisis intervention, and care coordination for individuals with co-occurring disorders. Services are provided in collaboration with a multidisciplinary Integrated Care Team and in accordance with CCBHC requirements, evidence-based practices, and all applicable regulatory and licensure standards.

Core Responsibilities

Clinical Assessment & Treatment

  • Conduct timely and comprehensive intake assessments, biopsychosocial evaluations, and suicide and substance use risk assessments using CCBHC-required and evidence-based screening tools.
  • Complete standardized screening tools (e.g., PHQ-9, PCL-5, C-SSRS, and SCARED) to support diagnostic clarification and ongoing clinical care.
  • Develop, implement, and regularly update individualized, person-centered treatment plans that address mental health, substance use and/or social determinants of health-related needs, in collaboration with clients and their Integrated Care Team.
  • Monitor client progress, outcomes, and risk indicators; adjust treatment interventions as clinically indicated.
  • Provide individual, group, and/or family psychotherapy using evidence-based modalities including Cognitive Behavioral Therapy (CBT), Motivational Interviewing (MI), and Integrated Dual Disorder Treatment (IDDT), or Supportive Counseling approaches as needed.
  • Deliver substance use counseling, relapse prevention interventions, psychoeducation, and recovery-oriented supports tailored to client needs and stages of change as needed.
  • Participate in crisis response activities, including on-site or telephonic/video support, in alignment with CCBHC crisis response expectations.

Integrated Care & Care Coordination

  • Collaborate closely with the multidisciplinary Integrated Care Team, including medical providers, targeted case managers, peer support specialists, and other behavioral health staff, to ensure coordinated and whole-person care.
  • Communicate regularly with clients and, when appropriate, families or identified community supports to promote engagement, retention, and adherence to treatment plans.
  • Coordinate referrals and warm handoffs to internal CCBHC services and external providers, including inpatient psychiatric care, detoxification, residential treatment, and community-based supports.
  • Participate in interdisciplinary collaboration with prescribing providers to support integrated, person-centered treatment planning in accordance with CCBHC standards.
  • Engage in community outreach and partnership activities consistent with CCBHC access and continuity-of-care requirements.
  • Assist with social determinants of health-related needs as needed.

Documentation, Quality & Compliance

  • Complete accurate, timely, and compliant clinical documentation (assessments, treatment plans, progress notes, discharge summaries) in accordance with CCBHC standards, clinic policies, and state and federal regulations.
  • Ensure all services are delivered within the clinician’s scope of practice and consistent with licensure, ethical standards, and agency policies.
  • Participate in utilization review, quality improvement activities, and data-driven care initiatives as required by CCBHC oversight.

Professional & Team Responsibilities

  • Participate in regular clinical supervision, case reviews, interdisciplinary team meetings, and integrated care planning forums.
  • Contribute actively to multidisciplinary treatment discussions and shared decision-making.
  • Maintain professional competency through ongoing training, continuing education, and participation in workshops, conferences, and professional networks
  • Uphold positive, respectful and supportive attitude with colleagues and client.
  • Represent agency in a professional manner during and outside of work hours.
  • Support organizational and CCBHC program goals by adapting to evolving service needs and contributing to program development.
  • Perform other duties as assigned.

Professional Development & Tiered Advancement

Tier I – Limited Permit / Permit-Eligible

  • Master’s-level clinicians who are eligible for, or currently hold, a NYS limited permit
    • Provide services under close supervision while developing competency in integrated, co-occurring disorder treatment.
    • Focus on skill development and progression toward independent licensure.
    • Will participate in weekly clinical supervision.
Tier II – Licensed (licensed professionals, without an advanced clinical license)

  • Clinicians with initial licensure (e.g., LMSW, LMHC, LMFT, LCAT, LIC, or equivalent)
    • Practice with increasing clinical autonomy under ongoing supervision within the CCBHC framework.
    • Function more independently, within scope, begin to contribute to program development, and provide mentorship and clinical support to Tier I staff.
    • Adequate knowledge of DSM-5-TR and ability to formulate diagnostic impression for mental health and substance use related disorders.
    • Will participate in, at minimum, bi-weekly clinical supervision.
Tier III – Advanced Clinically Licensed

  • Clinicians with advanced licensure (e.g., LCSW, LMHC-D, or equivalent)
    • Function fully independently, within scope, contribute to program development, and provide mentorship and clinical support to Tier I and Tier II staff.
    • Working knowledge of DSM-5-TR and ability to independently diagnosis mental health and substance use related disorders.
    • Will participate in, at minimum, monthly clinical supervision.
Required Qualifications

  • Master’s degree in Social Work, Mental Health Counseling, Marriage and Family Therapy, or a related field.
  • Active NYS licensure, limited permit, or ability to obtain required credentialing within 90 days of hire.
  • Knowledge of evidence-based practices, trauma-informed care, and integrated treatment models.
  • Strong communication, organization, and documentation skills.
  • Ability to function effectively within a multidisciplinary, integrated care setting.
  • Valid New York State driver’s license.

Preferred Qualifications

  • Experience in CCBHC, community behavioral health and/or substance use treatment, hospital setting or integrated care setting.
  • Experience working with individuals living with a serious mental illness and/or substance use disorder.
  • Demonstrated crisis intervention and de-escalation skills.

Work Schedule & Flexibility

This Position Offers a Flexible Schedule Designed To Support Work–life Balance While Meeting Client And Program Needs. Schedules May Be Arranged Using Any Combination Of The Following Hours, Based On Mutual Agreement

  • Weekday daytime hours: Monday–Friday, 8:00 a.m. – 4:30 p.m.
  • Evening availability (select days): Tuesday, Wednesday, and/or Thursday, up to 8:00 p.m.
  • Occasional Saturday coverage: First and third Saturday of each month, 8:00 a.m. – 4:30 p.m.
