Client Field Care Coordinator

Summary:
At Upscale Home Care, we are dedicated to providing personalized, compassionate care that supports dignity, independence, and peace of mind for our clients. As a Client Field Care Coordinator, you will be going into member's homes and preforming the ATC monitoring visits. This role includes resolving conflicts, reassigning caregivers as needed, training caregivers as needed, handling client/caregiver inquiries, communicating with case managers, and any other field work that is needed.
Responsibilities:
- Preform ATC visits with members according to AHCCCS regulations.
- Respond to phone, email, and text inquiries in a timely and efficient manner.
- Monitor caregiver performance and provide hands on training as needed.
- Match and schedule caregivers with clients' needs if needed.
- Monitor caregiver schedules and confirm missed visits.
- Provide compassionate and respectful support to clients and caregivers.
Requirements:
- Outgoing, compassionate, tactful, detail-oriented, and able to multitask.
- 1 years of experience in a related field.
- Ability to work in a fast-paced environment and prioritize tasks effectively.
- Vehicle in good working order with valid registration and insurance.
Compensation:
- Full-time, in-person, hourly position (based on experience)
- Sick time off after 90 days and PTO after 1 year of hire
- Paid legal holidays off
