- 11:00 PM – 7:00 AM
Job Overview
Autumn Lake Healthcare is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our Long-Term Care team. The LPN will deliver quality nursing care to residents with chronic conditions, disabilities, and long-term healthcare needs while promoting comfort, dignity, and safety.
Key Responsibilities
- Perform comprehensive resident assessments and monitor health status
- Develop, implement, and evaluate individualized care plans
- Administer medications and treatments per physician orders
- Document vital signs, treatments, and changes in condition accurately
- Provide emotional support and education to residents and families
- Collaborate with interdisciplinary healthcare teams
- Maintain compliance with clinical and regulatory standards
Qualifications
- Active LPN license in Maryland or Compact State
- Long-term care or geriatric nursing experience preferred
- Strong clinical, communication, and documentation skills
- Full 100 % Credentials is a Must to get hired.
