Upper Valley Services (UVS) Developmental Disabilities Services & Adult Family Care Full-Time | Exempt Flexible Schedule | Hybrid Opportunity Location: Randolph, VT and Bradford, VT (Office-Based with Home Visits) | Up to 2 Days Remote per Week

As a Nurse Consultant, you will provide clinical oversight, training, consultation, and health coordination for individuals receiving services through our Developmental Disabilities Services and Adult Family Care programs. Position Summary

The Nurse Consultant is responsible for assessing, monitoring, training, and supporting the healthcare needs of individuals served through UVS Developmental Disabilities Services and Adult Family Care programs. This role ensures compliance with Vermont Developmental Disabilities Services Division (DDSD) Health and Wellness Standards, agency policies, and State regulations while providing clinical expertise to staff, shared living providers, and (removed interdisciplinary) teams.

Essential Responsibilities

Clinical Oversight & Consultation

  • Provide nursing consultation and support regarding healthcare needs of individuals receiving services.
  • Review medications and physician orders to ensure alignment with the agency's electronic health record system (CareLogic).
  • Review Medication Administration Records (MARs) monthly and provide feedback as needed.
  • Monitor compliance with DDSD Health and Wellness Standards, agency policies, and State regulations
  • Collaborate with service coordinators and teams regarding health concerns, medication issues, and healthcare planning.
  • Participate in home visits and team meetings as needed.
  • Provide nursing consultation for individuals with complex or emerging medical needs.
  • Provide nursing coverage for other UVS nurses during planned and unplanned absences.

Training & Education

  • Maintain Adult/Child/Infant CPR and AED Instructor certification.
  • Conduct CPR and AED trainings throughout the year.
  • Provide General Medication Administration Training for staff, shared living providers, and contracted workers.
  • Provide Bloodborne Pathogen and Infection Control training.
  • Educate teams regarding Vermont Department of Health and CDC recommendations and precautions.

Delegation & Documentation

  • Delegate medication administration and special care procedures as required by Vermont standards.
  • Complete self-medication assessments and delegations.
  • Document nursing notes, physician communications, medication delegations, and special care procedures in CareLogic.
  • Maintain timely, accurate, and compliant documentation.
  • Promote infection control practices and universal precautions.

Compliance & Team Collaboration

  • Follow all UVS, state, and federal regulations, policies, and procedures.
  • Complete required onboarding, annual, and ongoing training requirements.
  • Participate in team meetings and nursing team collaboration.

Qualifications

Required

  • Active, unrestricted Vermont Registered Nurse (RN) license.
  • Minimum of one year of nursing experience.
  • Strong communication, organizational, and documentation skills.
  • Working knowledge of Microsoft Outlook, Teams, Word, and other computer applications.
  • Ability to learn and utilize electronic health record systems.
  • Ability to maintain confidentiality.
  • Ability to work independently while collaborating effectively with interdisciplinary teams.
  • Commitment to community inclusion, person-centered supports, and individual choice.

Preferred

  • Experience supporting individuals with developmental disabilities.
  • Experience in Adult Family Care, case management, community health, or related settings.

Schedule

  • Full-time exempt position.
  • Flexible scheduling available.
  • Includes participation in an on-call rotation.
  • On-call responsibilities may include evenings, weekends, holidays, and emergency coverage.
  • On-call schedule will be coordinated with the Lead Nurse Consultant.

Travel Requirements

  • Travel throughout the service area is required based on program needs.
  • Valid driver's license required.
  • Reliable transportation required.
  • Must maintain personal automobile liability insurance at or above Vermont state minimum requirements.

Work Environment

  • Primarily office-based with periodic community and home visits.
  • Hybrid work option available, with up to two remote days per week upon supervisor approval.
