Upper Great Lakes Family Health is seeking a MAT Care Manager to provide care coordination and support to patients enrolled in the Medication-Assisted Treatment (MAT) program for substance use disorders. This role focuses on improving patient engagement, treatment adherence, and recovery outcomes through patient-centered care, collaboration with multidisciplinary teams, and consistent monitoring and follow-up.

PROFESSIONAL/TECHNICAL KNOWLEDGE, SKILLS & ABILITIES
- Minimum of two years’ experience in addiction medicine, behavioral health, or care management.
- Demonstrates an understanding of FQHCs.
- Demonstrates an understanding of PCMH model of care.
- Knowledge of substance use disorders, recovery models, and MAT protocols (e.g., buprenorphine, naltrexone, Sublocade, Vivitrol, Brixadi).
- Proficient in motivational interviewing and trauma-informed care.
LICENSES & CERTIFICATIONS
- Current unrestricted Michigan Registered Nurse (RN) or Licensed Practical Nurse (LPN), or equivalent clinical credential.
- CPR-BLS Certification
TECHNICAL SKILLS
- Microsoft Office Suite to create correspondence, tables, charts, graphs, spreadsheets, and presentations.
- Adobe Acrobat to create and manipulate forms.
- Electronic clinical systems, including EHR and scheduling systems in use at the health center.
