What are the responsibilities and job description for the Licensed Practical Nurse - Community Treatment Team position at UPMC?
The Licensed Practical Nurse (LPN) participates in the planning, implementation and evaluation of nursing care using focused assessment in settings where nursing takes place under the direction of a Registered Nurse or other licensed UPMC provider. In this role, the LPN must take nurse assignment at the top of their license in a med/surg/tele or Emergency Department setting.
Responsibilities:
Clinical Responsibilities
- Provides patient care using sound nursing judgment and focused assessments.
- Seeks RN guidance when care exceeds LPN scope or competency.
- Follows UPMC policies, LPN guidelines, and State Practice Act.
- Develops daily goals and holistic care plans with patient involvement.
- Documents thoroughly to support care coordination.
Teamwork & Communication
- Collaborates with healthcare team to ensure quality outcomes.
- Builds compassionate relationships with patients, families, and peers.
- Actively supports unit activities and assists team members.
Education & Growth
- Educates patients and families for smooth care transitions.
- Mentors new LPNs and support staff.
- Pursues feedback and continuous improvement.
Engages in learning programs and quality initiatives.
