UPMC Home Health is hiring a Full-Time Physical Therapist for the Williamsport and surrounding areas.
Daylight hours: 8:00 am- 4:30 pm with rotating on-call weekends as needed.
Purpose:
Provide age-sensitive professional physical therapy services in the home and community setting, including assessment, intervention, treatment, and instruction to individuals of all ages to maximize functional abilities and ensure optimal care.
Responsibilities:
- Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.
- Continually develops professional skills by assuming responsibility for ongoing continuing education.
- Evaluates, plans, and treats patients in an effective Physical Therapy Program including the use of modalities and assistive devices within the Standards of Physical Therapy Practice.
- Identifies, develops, and participates in process improvement opportunities within the Agency's Home Health or Early Intervention programs that will enhance the quality of services we provide.
- Assigns and supervises the Certified Physical Therapy Assistant, if applicable.
- Attends compliance training and adheres to the organization's standards of conduct, policies, and procedures.
- Performs duties and job responsibilities in a fashion that coincides with the service management philosophy of UPMC towards patients, visitors, staff, peers, physicians, and other departments within the organization.
- Maintains and completes all required documentation, orders, Point of Care or written, promptly and accurately within Agency and regulatory standards. Data syncs (transmits) information the same day, if applicable.
- Advises and consults with the family, inter- and intradisciplinary team members, physicians, and other professionals, as appropriate, to ensure optimal patient care.
- Provides physical therapy services and participates in the development of a physical therapy plan of treatment, including modalities, frequency, and treatment, in collaboration with the referring physician for patients from birth to geriatrics to assist patients in obtaining maximum functional independence within the standards of physical therapy.
