UPMC Home Healthcare and Family Hospice is hiring a Home Care Aide to support Erie County. This is a Monday through Friday, 8:00 AM-4:30 PM position with rotating weekend shifts, providing exceptional care to patients in their place of residence. This is a field position, making home visits to both home health and hospice patients.
Responsibilities:
- Communicates appropriately with the interdisciplinary home care team promptly, reporting any changes in the patient's condition to the appropriate professional staff.
- Attends compliance training and adheres to the organization's standards of conduct, policies, and procedures.
- Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.
- Provides safe and direct personal care to patients in the home setting under the direct supervision of appropriate professionals.
- Performs duties and job responsibilities in a fashion that coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians, and other departments within the organization.
- Identifies, develops, and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide.
- Proven ability to facilitate change.
- Maintains and completes documentation accurately and promptly while in the patient's home and within the agency and regulatory standards.
- Participates in all required agency meetings.
- Must maintain annual competency evaluation scores as determined by regulatory requirements
