UPMC Presbyterian is looking to hire a Full-TIme Clinical Transplant Coordinator II to support Kidney Transplant. This position will support the living donor team. 

Schedule:

  • Monday - Friday
  • Daylight hours

Purpose:


The Clinical Transplant/VAD Nurse Coordinator II has the authority, responsibility, and accountability for the delivery of transplant/VAD nursing care through the use of the transplant/VAD nursing process. The Clinical Transplant Coordinator II, in collaboration with the Supervisor, Clinical Transplant/VAD Coordinators, transplant/VAD team and other health care professionals, applies competent clinical knowledge and skills to achieve quality care outcomes.The Clinical Transplant/VAD Coordinator II must demonstrate the knowledge and skills necessary to provide care that is relevant to patients under their care, as well as apply principles of growth and development over the life span. The Clinical Transplant/VAD Coordinator II also must possess the ability to assess and interpret patient data needed to identify each patient's requirements relative to his or her age specific needs.

Responsibilities:
 

  • TRANSPLANT/VAD REFERRAL AND EVALUATION: 1. Identifies appropriate transplant/VAD candidates upon referral and interacts with appropriate hospital staff. 2. Identifies requirements for candidate evaluation. 3. Coordinates the evaluation process for potential transplant/VAD candidates with the multidisciplinary team. 4. Functions as candidate advocate by applying knowledge of growth, development, educational, and cultural background during candidate and family interaction. 5. Provides data to regulatory agencies. 6. Identifies absolute and relative contraindications to transplantation as applicable. 7. Demonstrates knowledge of, and compliance with, United Network for Organ Sharing (UNOS) polices and listing requirements. 8. Acts as resource person for internal and external health care providers to optimize the patient's condition during the waiting period. 9. Develops ongoing rapport and relationship with referring physician and other health care providers to enhance and maintain the referral base. 10. Participates in patient and staff education with the established referral base. 11. Develops and provides education programs to the community, professional organizations, and local health care providers to increase referral base.
  • PROFESSIONAL GROWTH AND DEVELOPMENT: 1. Maintains ABTC Certification. 2. Reviews and integrates into practice the current professional literature. 3. Attends practice-related conferences. 4. Participates in professional specialty-related organizations. 5. Examines current transplant/VAD-related research. 6. Shares knowledge through mentoring newly hired Transplant/VAD Coordinator I. 7. Participates in and supports ongoing clinical research within the transplant/VAD department. 8. Constructively receives feedback and direction. 9. Identifies and communicates learning needs to manager. 10. Serves as a resource for staff and other disciplines. 11. Initiates participation in project and committee work. 12. Takes action to improve knowledge, skills and performance based on feedback or on self-identified developmental needs. 13. Uses problems as challenges and helps peers to do likewise. 14. Participates in self-review as requested by manager. 15. Adheres to On-Call schedule as required.
  • LEADERSHIP: 1. Participates in the hiring process, providing feedback as part of a collaborative process in conjunction with Supervisor, Clinical Transplant/VAD Coordinator and Director. 2. Orients, trains, and develops staff. 3. Influences peer behavior to create positive change. 4. Suggests and supports changes within the department. 5. Applies creative and innovative approaches in the provision of patient care. 6. Assumes charge responsibilities in clinic. 7. Assumes responsibilities of Supervisor, Clinical Transplant/VAD Coordinators in his/her absence. 8. Participates/chairs department committees .9. Participates in UPMC based committees. 10. Actively participates in professional organizations and activities. 11. Represents the Transplant Services/ UPMC at local, national, and international transplant/VAD meetings.
  • PRE-TRANSPLANT/VAD PERIOD: 1. Develops and institutes a teaching plan addressing transplant/VAD candidates learning needs. 2. Maintains communication with patient and referring physician. 3. Identifies the need for candidate re-evaluation and initiates when appropriate. 4. Provides and maintains comprehensive documentation of candidate process. 5. Acts as resource person for internal and external health care providers. 6. Exhibits ability to maintain and update candidate-listing status per United Network for Organ Sharing (UNOS) Regulations. 7. Recognizes potential problems, abnormal diagnostic findings and/or significant changes during the waiting phase, coordinating required care.
  • PERI-OPERATIVE PERIOD: 1. Participates in transplant/VAD process according to UPMC protocol. 2. Reviews potential recipient's current medical status to determine eligibility for transplant/VAD. 3. Facilitates admission or transfer to UPMC. 4. Communicates with organ procurement agency/ORC and collects the appropriate data necessary for decision making re organ acceptance/rejection for transplant/VAD. 5. Verbalizes understanding of standing orders and acts as resource for internal and external health care providers. 6. Exhibits ability to remove transplant/VAD recipient from list per UNOS Regulations as applicable.
  • POST-TRANSPLANT IN-PATIENT PERIOD: 1. Monitors patient progress. 2. Educates recipient and family regarding care and responsibilities after transplantation. 3. Collaborates with transplant/VAD team to establish and implement patient plan of care. 4. Communicates recipient status to referring physician, health care providers, and payors. 5. Acts as a resource for internal and external health care providers.
  • POST-TRANSPLANT/VAD OUT-PATIENT PERIOD: 1. Identifies and responds to recipient and family needs in post-transplant/VAD phase. 2. Reinforces educational and follow-up requirements. 3. Collects and reviews data pertinent to recipient's health and organ function. 4. Provides and maintains comprehensive documentation of recipient's progress. 5. Implements changes in drug therapy as ordered by physician. 6. Arranges for consultations, diagnostic procedures, and hospitalizations when indicated. 7. Maintains communication with referring physician and other health care providers. 8. Assesses recipient's compliance with medications and medical regime; collaborates with transplant/VAD team to attain and maintain compliance. 9. Assumes responsibility for complex patient assignments. 10. Verbalizes understanding of post-transplant protocol and acts as resource for internal and external health care providers. 11. Provides and maintains comprehensive documentation of recipient progress. 12. Reinforces and facilitates the need for well care. 13. Monitors the recipient's risk related to short- and long-term complications of immunosuppression.
