Primary Function:
Under supervision of the Chief of Home Health Services, the HHA Nurse Supervisor conducts HHA interviews and testing, introduction to cases, routine clinical supervision, education, and provides overall support to the aides. Services will be provided in conjunction with the Home Care and Elder Services Plan/PACE Teams. HHA clinical supervision will be provided according to governing regulatory requirements, HHA skills performance and documentation standards. A full orientation is provided.
Pay Range: The expected base pay for the position is $40 - $71 per hour (commensurate with years of experience) and may be increased based upon other factors, such as language, certifications, etc.
Duties & Responsibilities:
• Responsible for hiring, orientation, training and onboarding of new HHAs.
• Supervise and direct all HHA services for Elder Services/PACE patients. Supervision includes home visit reviews at regular intervals to all patients receiving Home Health Aide services. Home visits are to supervise and evaluate services provided.
• Conduct initial home visits [Introductions] to introduce HHAs to new patients and to the plan of care.
• Conduct Supervisory Home Visits at regular intervals to patients receiving HHA services.
• Work with Admin Coordinators to ensure daily proactive and efficient scheduling of HHAs for daytime, evening and weekend case assignments considering patient needs and preferences in conjunction with HHA availability and skills.
• Work with Admin Coordinators and HHAs to ensure proactive and timely communication of HHA schedule changes and management of case coverage related to HHA planned or unplanned absences.
• Monitor the flow of information from HHAs’ Daily Activity Sheets into the patient’s records and assure staff compliance Time Cards [Paycom] and with record keeping protocols.
• Schedule, develop and conduct in-service programs and case conferences for HHAs.
• Provide direct patient care as necessary during home visits.
• Conduct annual individual HHA performance reviews and skills assessments.
• Field HHA calls from patient homes and facilitate an appropriate response or resolution to the HHA calls.
• Assist to investigate, resolve and document complaints and incidents related to HHA services.
• Act as a back-up to the HHA Services Team’s Call-Out line.
• Responsible for staff compliance with regards to requirements for billing, personnel policies, patient grievances, incident reports and other regulatory mandates.
• Must average 20-25 client visits per week (for full-time hours).
• Handle other responsibilities as assigned by the Chief of Home Health Services.
Minimum Basic Knowledge:
• Licensed Registered Nurse in the Commonwealth of Massachusetts required; B.S.N. degree preferred.
Experience & Qualifications:
- Two years of nursing experience, with at least 1 year in home health care/or long-term care.
- At least 1 year of supervisory experience, preferably in a clinical setting.
- Bilingual language skills a plus; additional fluency in Spanish, Cape Verdean Creole and/or Portuguese preferred.
Essential Functions:
- Clinical supervision to HHAs every 60 days for CMS and 14 days for Massachusetts Department of Public Health (per CMS/DPH mandate).
- Hold an average of 10-15 supervisions/week (based on a current total of 94 Home Health Aides).
