Schedule : Monday to Friday 8 am - 4:30 pm
Pay Range: The expected base pay for the position is $20-$28 per hour and may be increased based on other factors, such as language, certifications, etc.
Primary Function:
Under the direction of the Director of Home Health Aide Services & Business Operations, the Home Care/HHA Coordinator is responsible for:
A. Acting as a Member of the Administrative Team:
Scheduling and data entry tasks to update the scheduling system; timely filling of cases and HHA coverage for daily, vacation or holiday shifts; provide coverage for other administrative staff during planned or unplanned absences, and act as a point of contact for HHA services.
B. Directly schedule Home Health Aides/Home Makers:
Deployment, assisting with/verifying schedules/visits and answering calls.
Duties & Responsibilities:
- Collaborate with other administrative staff and the Director to ensure timely placement of cases by optimizing Electronic Scheduling System.
- Communicate with Home Health Aide Nursing Supervisors regarding patient intros and HHA supervisions.
- Collaborate with administrative staff to ensuring daily communication on HHA assignments for PACE and Home Care Clinical Teams.
- Provide cross-coverage duties for other administrative staff planned or unplanned absences.
- Schedule staff (HHA, clinicians) considering patient needs and preference in conjunction with creating and overseeing schedules in Wellsky (EVV) & PCA.
- Communicate with patient families regarding HHA/clinician schedules and schedule changes related to sick calls or PTO coverage.
- Obtain Director’s approval for any Schedule changes before confirming changes with Home Care/PACE Teams and patient families.
- Assists with administrative orientation of HHAs to Home Health Aides Services, the Home Care Program, Upham’s Elder Service Plan and ASAP’s regarding this role.
- Assists with in-service preparation as needed.
- Monitors the weekly flow of Activity Reports for Paycom and Electronic Visit Verification for Home Care/SCO Billing. Verify schedules in Wellsky-PCA are in accordance with Paycom.
- Verify Insurance and proof of services for Home Care clients.
- Provide other supportive task for department: inbound/outbound calls, faxes, checking for referrals.
- Other responsibilities as assigned by the Director.
Minimum Basic Knowledge:
Must hold a high school diploma or equivalent in years of experience. Prefer some college level coursework or several years of professional work experience in fast-paced setting. Must Speak Spanish, Cape Verdean Portuguese or Haitian Creole.

Experience working with Home Health Aides and with senior citizens is a plus.
2. Must have excellent interpersonal skills, computer skills, including Microsoft Office products; Word, Excel & Outlook, and the ability to multi-task.
3. Must have cultural sensitivity and ability to work effectively with various cultural, ethnic, and linguistic backgrounds.
4. Knowledge of a second language is a MUST (Spanish preferred).
5. Maintain a high level of customer service and professionalism towards all patients, families, colleagues, including Home Health Aides, Team/Business Partners in PACE/Home Care/ASAP’s etc., when managing and fielding calls from Patients/Participants/Family Members etc.
