PortagePointe is a long-term care facility with 60 beds to offer to the senior community and is known as the premier nursing home serving the entire Upper Peninsula. We are a certified Eden Alternative facility and take great pride in providing a home-like environment to residents. PortagePointe is regulated by the Centers for Medicare and Medicaid (CMS), a member of the American Health Care Association (AHCA), and a member of the Healthcare Association of Michigan (HCAM).

How You'll Contribute
A Post Acute RN who excels in this role:
- Performs plan of care intervention, including medication administration, specimen collection, clinical treatments, as well as other medical care treatment.
- Documents patient care given.
- Monitors, records, and communicate patient condition as appropriate to care team, physician, patient, and family.
- Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
- Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs.
- Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstances.
- Works as an advocate for the physical and emotional well-being of the patient.
- Nurtures, sustains, and protects each Elder by providing the services needed to live a full life.
- Incorporates the Eden Philosophy of living into every action undertaken throughout one's employment.
- Encourages each Elder to reach and maintain his/her maximum functional ability.
- Creates opportunities for the Elder to give care to others.
- Committed to the development of the staff, Elders, and families as collaborative team members. Teamwork is a prime responsibility for everyone.
- Performs other job duties as assigned.
