How you will contribute
A Clinic Case Manager who excels in this role will:
- Identifies the targeted population and implements clinical interventions and protocols.
- Collaborates with PCP, patients and members of health care team including continuum of care settings and community resources.
- Provides self-management support and includes patient/family as active members in the team approach.
- Facilitate close monitoring of high-risk patients to prevent and/or intervene early during acute exacerbations.
Coordinates care between PCP and acute care hospitals, specialists and post -acute care services.

What we are looking for
Applicants should have a state of Michigan RN license and possess an associate degree from an accredited nursing school (bachelor's degree preferred).
Additional requirements include:
Basic Life Support Health Care Provider (BLS-HCP)
Two (2) years’ experience in adult medicine and pediatric patients.
