Direct Support Professional Job Description. Join our Adult Foster Care Team in providing hands on person-centered support to people with disabilities and mental health issues. The direct support professional will provide support to individuals by assisting them with their daily living needs in order for them to gain independence and lead fulfilling lives in the community.
DSP Responsibilities:
Attends and completes all required trainings
Participates in community activities with individuals
Assists with self-cares such as bathing and dressing
Prepares meals and or assists the individual with meal preparation
Administers medication and completes documentation on the Medication Administration Record (MAR)
Monitors health care needs of the individual and communicates directly with the registered nurse
Documents detailed narratives and behavior data daily
Monitors and maintains safety of individuals providing direct care
Follows programming outlined in the Individual Abuse Prevention Plan (IAPP), and Coordinated Services and Support Plan and Addendum (CSSP)
Models behavior and mentors individual to increase their daily living skills
Communicates and works alongside guardians, case managers, families and other team members
Collaborates with co-workers in a team approach to provide quality care to individuals
The ability to de-escalate crisis situations and behaviors that may require physical intervention on occasion

Hours: Part time (Week A) Sunday 9am-9pm, Monday, Tuesday 4pm-9:30pm, Wednesday, 9am-4pm or 4pm-9:30pm, (Week B) Wednesday,9am-4pm or 4pm-9:30pm, Thursday and Friday, 4pm-9:30pm, Saturday 9am-9pm Full Time: (Week A) Sunday, Monday, Tuesday, 9am-9pm, Wednesday 9am-4pm or 4pm-9:30pm, (Week B) Wednesday 9am-4pm or 4pm-9:30pm, Thursday-Saturday, 9am-9pm. Dates and time could change to meet needs of individual sites.
Qualifications:
Must be 18 years of age
Must possess a valid driver's license (required) and must have a clear driving record.
Basic domestic skills (cooking, cleaning, laundry).
Ability to effectively communicate in English, both written and verbally
