Job Overview
We are seeking a dedicated Licensed Vocational Nurse (LVN) to join our Skilled Nursing Facility (SNF) team. The LVN will provide high-quality patient care, assist with medical assessments, and support the healthcare team in delivering comprehensive long-term and acute care. The ideal candidate will possess strong clinical skills, experience with diverse patient populations, and proficiency in electronic health record systems. This role offers an opportunity to work in a dynamic environment focused on improving patient outcomes across various care settings, including geriatrics, memory care, and post-acute rehabilitation.
Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation.
- Administer medications and injections accurately following physician orders while adhering to HIPAA and infection control protocols.
- Manage IV infusions, catheter care, tube feeding, and ventilator support as required.
- Document patient care activities thoroughly using EMR/EHR systems such as Epic, Cerner, or Athenahealth.
- Assist with diagnostic evaluations, specimen collection/processing, and medical imaging procedures.
- Provide specialized care for patients with Alzheimer's disease, dementia, developmental disabilities, or behavioral health needs.
- Support discharge planning, case management, and coordination of post-acute or home & community care services.
- Ensure compliance with safety standards including aseptic technique and infection control measures.
- Collaborate with physicians and multidisciplinary teams in emergency medicine, ICU, trauma centers (Level I & II), or pediatric settings such as NICU or PICU when necessary.
- Educate patients and families on health coaching, medication management, nutrition, and chronic disease management.
Pay: $2,170.00 per week
