Job Description: The Pharmacist (Clinical) optimizes medication use by the provision of pharmaceutical care to patients. The provision of such care entails that the appropriate medication is used, in the correct dosage, with appropriate monitoring for clinical effect and the absence of adverse effects. The patient’s medication profile is reviewed to assess possible significant drug interactions and inappropriate indications. Drug information is provided in an accurate and timely manner, not only to health professionals, but the public as well. Components of this position includes (but is not limited to) direct supervision of pharmacy technicians and clerical personnel, and service on various interdisciplinary committees. Participates in the educational programs of the Pharmacy Department, anticipated pharmacy residents and pharmacy students, supervising the training for pharmacy technician students, and offering in-service to staff. Duties/Responsibilities: Demonstrates ability to adequately design, recommend, monitor and evaluate patient specific pharmacotherapeutic regimens when consulted. Provides accurate information on medication use, possible side effects and their management, drug interactions or other needed facts to patients and their caregivers. Provides accurate, timely drug information to other healthcare providers. Responds to cardiopulmonary arrests and other medical emergencies and provides accurate, timely emergent care. Assists with the design of medication usage evaluations and corresponding data collection. Demonstrates proficiency with the process for assessing, managing, preventing, and reporting medication errors. Demonstrates proficiency with the process for assessing, managing, preventing, and reporting adverse reactions to medications. Demonstrates proficiency with the drug formulary policies and procedures. Prepares and dispenses medications using appropriate techniques and following departmental policies and procedures. Manages workflow well, utilizing departmental staff to provide timely, efficient pharmaceutical care. Demonstrates expertise and teamwork on patient care committees, aiding the interdisciplinary team to find solutions to medication use issues. Education: Graduate of an accredited college of pharmacy, possessing either a BS in pharmacy or Doctor of Pharmacy required. Experience: None. Institutional practice experience preferred. Licensure/Certification: A license to practice pharmacy in the state of Kentucky required.
