The Prevention Program is funded through the Administration for Childrens Services provides comprehensive home and community-based preventive services to children and their families. The Program integrates Mobility Mentoring, an evidence-based model that is grounded in science emphasizing a growth mindset, high expectations and belief and investment in all children and families.

Case Planners work directly with children and families supporting their stability and growth and the accomplishment of family identified goals and will integrate Mobility Mentoring into all aspects of their work. The Program works collaboratively with other University Settlement programs, community-based partners and ACS to ensure children and families have access to and are linked with resources and services that help them problem solve and support families in reaching their optimum goals and achievements.

Responsibilities

  • Maintain weekly contact with a caseload of families in the home/community/office in accordance with program contract
  • Complete safety and risk assessments in accordance with ACS contract mandates
  • Integrate Mobility Mentoring into work with all families, including completing relevant documentation showing efficacy to the model
  • Attend all required trainings as required and integrate training into work with families
  • Provide casework counseling through a trauma informed framework that takes into consideration the impact of poverty, oppression and structural racism and continually assesses the family, the familys level of engagement in services, parental motivation in relation to goals and the level of in-home family functioning & stability
  • Provide mediation and/or crisis intervention when indicated
  • Participate in Family Team Conferences, weekly supervision, weekly team/staff meetings and in-service training
  • Maintain collaborative relationships with ACS Field offices, medical providers, schools and other community partner organizations to ensure that families receive appropriate referral and linkages to other service providers
  • Document all related information
  • Ensure entries into CONNECTIONS & PROMIS are up-to-date, accurate and submitted within deadline
  • Prepare all assessments and progress notes, in a clear and effective manner, in accordance with agency protocol & within established time-frames
  • Actively engage the child(ren), family and/or guardians in case planning, setting goals, establishing priorities and collaboratively tracking progress within the Mobility Mentoring framework to proactively support and ensure progress toward desired outcomes
  • Initiate solutions to eliminate barriers to goal achievement
  • Contact families within 24 hours of referral to the program to schedule initial intake and meet with families within five (5) days of referral
  • Assist families with concrete needs and services, educational advocacy and referrals to support services, rent arrears, utility shut off, etc.
  • Monitor school attendance, obtain school records and intervene with DOE
  • Other tasks as assigned by supervisor

Qualifications

  • Bachelors degree required in preferred Social Work or other human service field
  • Two years of related experience working with children and/or families preferably in a child welfare, juvenile justice or mental health settings
  • Good organizational skills with ability to meet deadlines
  • Work flexible hours including evenings and some weekends as needed
  • Ability to work independently, as well as contribute to a team
  • Strong clinical skills and ability to work under pressure with high risk, crisis-oriented cases
  • Very good interpersonal/communication skills, including ability to write well and meet copious amount of documentation in a timely manner
  • You have a strong commitment to advancing diversity, equity, and inclusion
  • Bilingual Spanish/English fluency is a plus
  • Possess computer skills and willing to learn new systems

Work Schedule: Full time, Monday - Friday, Flexible schedule, Hybrid, 35 hours per week
