Role Description
This is a full-time, on-site role for a Case Manager - Wellness Together based in New York, NY. The Case Manager will work directly with clients to assess their needs, develop personalized service plans, and provide ongoing support to enhance their overall well-being. Duties include maintaining comprehensive case files, conducting regular check-ins, coordinating care with service providers, linking clients to resources such as housing, healthcare, and education, and addressing barriers to accessing services. Collaboration with community partners and support in the program's administrative tasks will also be key responsibilities.
Qualifications
- Experience in Case Management or Social Work, including assessment, service planning, and coordination
- Strong interpersonal and communication skills to work effectively with diverse populations
- Organizational and documentation skills, including maintaining accurate case files
- Familiarity with community resources, public benefits, and referral networks
- Problem-solving abilities and a solution-oriented approach to addressing client needs
- Proficiency in a second language is preferred but not required
- Bachelor’s degree in Social Work, Human Services, or a related field strongly preferred
- Passion for community empowerment and commitment to social justice
