What are the responsibilities and job description for the Registered Nurse - Ambulatory Pediatric Complex Care position at University of Vermont Medical Center?
Unit Description:
Complex Care is a new subspecialty that will be established at the Golisano Children's Hospital at UVM Health fall of 2026.
* Serves as a nurse team specialist in the coordination of care for children with medical complexity (CMC) at an appropriate intensity to accommodate each family's strengths and needs.
* Applies nursing scope of practice, and clinical experience and knowledge in the care of children with chronic health conditions and disabilities.
* Coordinates the care and services of selected patient populations across the continuum of illness; promotes effective utilization and monitoring of health care resources; and assumes a role with the interdisciplinary teams to achieve optimal patient-centered, clinical and resource outcomes.
* Proactively identifies and intervenes to address barriers to treatment, health, wellness, prevention, improvement and outcomes.
* Serves as an important link between the patient, the healthcare teams and community resources. Actively participates in mentorship, training, and process improvement within their assigned team.
EDUCATION
Minimum education: Registered Nurse and current Vermont License required.
EXPERIENCE
Minimum Experience: 2 or more years of clinical experience required. Experience with pediatric patients with complex medical conditions preferred. Experience as care coordinator/case manager preferred. Eligibility to utilize VITL and Epic health informatics systems preferred (usually defined as eligibility to be hired by those health systems, i.e. without unresolved, past history of exclusion from hiring).
On-call: Not required

Requirements:
- Current RN licensure or compact licensure recognized by the State of Vermont required.
- ADN required, BSN preferred.
- Appropriate experience in specific clinical area. Varies by unit.
