Position Summary:
Develops, organizes, and implements the total social work program of the UT Hemophilia clinic in accordance with Accepted Social Work Practice. Functions as a clinical social worker and as a member of an interdisciplinary team member. Manages all aspects of state and federal grants including procurement, invoicing, and budgeting.
Position Qualification:
Service
Develops and maintains good working relationships with clinic team members, in-patient, and out patient social workers/care providers.
a. Maintains contact with other departments i.e., admitting, day surgery, out patient surgery, physical therapy, dentistry, and infusion center.
Arranging and Participating in inter disciplinary conferences/meetings.
a. Demonstrates excellent verbal and written communication skills.
b. Serves as a liaison for other hemophilia centers.
Bills for services rendered.
a. Assures required HIPPA, patient consent forms, insurance card copy, and other necessary billing information is obtained and placed on clinic chart.
Confers with the clinical manager and staff nurses regarding identified patient and community needs.
a. Communicates needs of social work issues including insurance coverage (lack thereof), transpiration and coordination of care.
b. Appropriately completes home and school assessments.
c. Educates professional, co-workers, patients, families, and communities about hemophilia.
Determines social assessment of total patient needs and follow through with a plan for treatment.
a. Identifies psycho-assessment skills and evaluation credibility.
Grants.
a. Generates annual goals and objectives including updates prospectively to meet requirements of grantors.
b. Timely provides invoices for all grants
c. Sets and monitors budget in accordance with the grant reqirements.
d. Meets with hospital accounting monthyl to discuss budget of grants.
Manages the social work activities of the Hemophilia clinic in such a way as to accomplish institutional as well clinic objectives.
a. Demonstrates ability to organize and carry out institutional and clinical objectives.
Provides direct social work services to individuals and families dealing with bleeding disorders and stressors.
a. Empathizes, shares, and promotes positive changes in those affected by bleeding disorders.
Enters data prospectively.
a. Generates grant and grant related reports, graphs etc.
b. Maintains records and required information.
