Job Overview
Manages a designated caseload to coordinate and complete timely assessments, planning, implementation and evaluation of discharge plans and care transitions across the continuum of care. Collaborates with medical staff, unit managers, and all professional care giver disciplines to plan and coordinate the plan of care with patients and families throughout the acute continuum of care and into post acute settings. Ensures optimum utilization of the patient's and the Health System's resources and perform these duties within the requirements of CMS and other external review agencies. Responds to Director/Manager as required.
Responsibilities
- Conduct thorough patient assessments to determine social needs, behavioral health status, and discharge requirements.
- Develop individualized discharge plans that include post-acute care options, community resources, and follow-up services.
- Coordinate with medical teams to facilitate discharge planning for patients in inpatient, outpatient, ICU, PICU, or emergency settings.
- Manage utilization review processes to ensure appropriate level of care and resource utilization aligned with insurance and hospital policies.
- Maintain accurate and confidential medical records in compliance with HIPAA regulations.
- Provide mental health counseling, addiction counseling, or behavioral health support as needed for diverse patient populations including pediatrics and adults.
- Collaborate with healthcare providers across various departments such as hospital medicine, emergency medicine, trauma centers, and outpatient clinics.
- Assist patients with intake, diagnostic evaluation, and connecting them to community resources for ongoing support.
- Participate in multidisciplinary team meetings to support comprehensive patient service delivery and facilitate smooth transitions between care settings.
- Stay current with hospital policies, medical terminology, physiology knowledge, and regulatory standards relevant to case management.
Qualifications
- Bachelor's or Master’s degree in Social Work (MSW preferred) or related field; licensure as required by state regulations.
- Proven experience in hospital environment, particularly within Level I Trauma Centers, ICU, Pediatrics, or behavioral health settings.
- Strong knowledge of medical terminology, discharge planning, utilization management, and post-acute care processes.
- Familiarity with HIPAA compliance, medical records management, and patient assessment techniques.
- Excellent communication skills for effective collaboration with healthcare teams and patients.

Pay: $50,000.00 - $65,000.00 per year
