$76,398.00 - $99,320.00The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
Responsibilities:
Provides individualized assistance, support and comprehensive coordination of care for patients and their families/caregivers, to eliminate barriers to timely care, facilitate flow through system, facilitate interactions with care team, increase patient and provider satisfaction, maintain point of contact with providers and serve as first point of contact for patients and families.ESSENTIAL FUNCTIONS
- Develops relationship with patients from time of diagnosis through initiation and completion of active treatment. Maintains collaborative relationships with families/caregivers, multidisciplinary provider(s), nursing leadership and staff, and other internal service delivery departments. Provides ongoing emotional support and coordination of care for patient.
- Ensures continuity of care. Communicates pertinent information regarding patient issues with other team members. Recommends services consistent with the patient’s care needs and benefits. Collaborates with schedulers to ensure all medical information is complete and available for visits, including pre-visit diagnostic results and manages efforts with goal of having measurable impact on improving patient’s overall quality of life.
- Coordinates patient education and care. Serves as ongoing resource for patients and families during diagnosis and treatment. Identifies patient needs and initiates appropriate referrals to supportive services/groups. Collaborates with other care providers regarding how to best meet the needs and sequence care. Assesses patient’s learning style and health literacy to provide education at the appropriate patient level. Develops community partnerships and resources to improve access to care.
- Participates in development of standards, implementation and evaluation of policies and procedures, development of compliance with treatment guidelines, and assessment of practice patterns, identifying needed changes and establishing measurable action plans.
- In partnership with data management staff, collects and reports data quality measures consistent with organizational needs. Other duties as assigned.
MINIMUM EDUCATION & EXPERIENCE
- Bachelor's degree in Nursing (BSN) and 3 years of Nursing experience required
- Or equivalent combination of education and experience If candidate is currently an Associates Degree RN, nurse has 5 years to complete BSN
KNOWLEDGE, SKILLS AND ABILITIES
LICENSES AND CERTIFICATIONS
- RN - Registered Nurse - State Licensure and/or Compact State Licensure upon hire required
- Certification in Nursing specialty area upon hire preferred
