Along with our physicians, we use a team of educators, pharmacists, and psychologists to teach patients how to prevent and manage disease.

21.62 - 31.35 USD Hourly

What You Will Do As a Community Health Worker
- Provide Direct Services such as wellness, health information, social services, disease prevention, chronic illness medication and social support. adherence to patients and providers
- May organize and/or facilitate support groups
- Refer and link to preventive services through health screenings and healthcare information
- Bridge the gap between communities and the health and social service systems by enhancing care quality by aiding communication between health system(s) and patient to clarify cultural practices and health information.
- Make referrals and coordinate services and enroll patients into programs such as health insurance and public assistance
- Partners with patient/care-giver/management in care/decision making.
- Community Health Worker Certificate at time of hire or completed within 6 months of hire
- 3 years’ experience working on community health projects, programs or initiatives.
