Responsibilities
- Plans, implements, and monitors population health programs that support the mission, values, goals, and objectives of the system and the local site as directed by manager or director.
- Assists with writing and/or purchasing collateral material to support population health initiatives.
- Represents local site and/or UMMS to local community partners to build relationships and collaborate on programing.
- Works on planning and development of projects with a variety of internal and external partners.
- Assists in the development and implementation of population health programs for targeted populations.
- Perform all other duties as assigned.
Education and Experience
- High school diploma or GED is required.
- One (1) to Two (2) years of relevant experience is required.
- Ability to manage, develop, plan, and deliver projects with minimal direction while meeting deadlines.
- Working knowledge of computer applications with experience in word processing and spreadsheet software such as Microsoft Word, PowerPoint, Excel, and CBISA.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work effectively with the public, community partners, vendors, and professional staff.
- Cultural competence to understand the various backgrounds of the individuals served.
- Ability to organize and analyze data on a computer.
- Empathy: health workers must have empathy to best serve their community
- Understanding of clinic operations, both inside and outside of strict healthcare settings.
- Weekend, shift work, holiday, on-call, may be required due to community related work
