Nurse Clinician (Nurse Navigator/Case Manager) - Burn - 100%

University of Iowa Health Care, Care Coordination Division is seeking a Nurse Clinician/Nurse Navigator to function as a clinical nurse expert for the Inpatient Burn Treatment Unit and as care coordinator for the assigned patient population facilitating seamless transitions across the continuum of care. The nurse functions as an administrative and clinical liaison for individuals to optimize quality health care delivery using clinical assessment/screening tools, education reinforcement, follow-up on the plan of care recommendations and guiding the utilization of cost-effective resources. This position will manage a specified population and work closely with t he interdisciplinary team which includes but is not limited to patient care staff, providers, patient support, family and community resources.

Duties to Include:

• Collaborate with nursing staff and other health care professionals in the development and review of standards and in planning, implementing and evaluating patient care for a specific patient population. Communicate with physicians and members of the health care team directing patient care.

• Provide direct nursing care, facilitate continuity of care and follow universal precautions. Coordinate and facilitate patient care between multiple disciplines including home health agencies, referring physicians and other community support services. Coordinate patient care to better utilize supportive services through patient visit and possible admission, pre-schedules testing, follows up with test results to both patient and physician, obtains medical history, coordinates follow up visit appointment, provides medication, rehabilitation teaching, etc.

• Serve as a clinical resource or consultant for specific patient population. Participate in development and implementation of managed care programs for a specific patient population.

• Provide leadership for direct patient care programs in area of specialization collaborating with unit-based nurse management team(s). Participate in the ongoing implementation of current regulatory standards in order to meet state and national agency accreditation.

• Design and implement patient/family health education activities for the patient population. Serve as an educational and clinical resource to patient/family/staff. Support the implementation and evaluation of staff education and orientation programs. Facilitate the educational experience for health care students and monitor the care delivered.

• Incorporate the role of patient care provider, advocate (patient, family, and health care professional), consultant, educator, researcher and quality manager.

• Use relevant nursing research to improve clinical practice and investigate nursing care problems.

Schedule : Monday through Friday, 8 hours shifts primarily between 7:00 AM and 6:00 PM. May require working holidays, based on divisional needs. May also require some weekends to cover vacation for weekend option staff.

Pay Grade: Pay Level 7-70

Required Qualifications

• A Baccalaureate degree in Nursing

• Minimum of 3 years of recent clinical nursing experience

• Current license to practice nursing in Iowa, or the ability to obtain licensure by the date of hire

• Experience working with an interdisciplinary team

• Excellent organizational skills, critical thinking, and problem-solving skills

• Excellent written and verbal communication skills

• Proficiency in computer applications including Microsfot Excel, Word, and PowerPoint

Desirable Qualifications

• Two years recent medical/surgical inpatient experience, including care coordination/case management

• Demonstrated expertise with patient/family discharge planning process

• Professional Master of Nursing and Healthcare Practice (MNHP), MSN/Clinical Nurse Leader or a Master’s Degree in Nursing (MSN, MA)

• Certification in case management

• Previous experience with EPIC
