This position is grant-funded for 12 months from hire date.
The position will work with community members from the Third Ward as part of the Wellness Hub – Third Ward, a collaboration between the UH Health Family Care Center, the Tilman J. Fertitta Family College of Medicine, and community partners. CHWs connect community members to medical and behavioral health services, food, and other social needs. The work will be conducted from a Third Ward community setting and the UH Health Family Care Center.
Candidates for this position must be able to conduct community visits in person and must have dependable transportation. Mileage for work-related travel is reimbursed. This position is in-office from 8 AM to 5 PM, with evening and weekend events. Advance notice will be given for evening and weekend assignments. The position is grant-funded for 12 months from hire date.
Preferred Experience: Case management and resource navigation experience preferred. Knowledge and experience with Third Ward preferred.
Helps move program efforts forward with the UH College of Medicine Community Health
Worker Initiative by providing outreach, navigation and eligibility assistance for participants of various programs.

- Serves as point of contact between community/neighborhood and the UH College of Medicine Community Health Worker Initiative.
- Establishes rapport with community members and families participating in the UH CHW initiatives.
- Recruits program participants for health programs.
- Conducts home/site visits to identify needs for services and provide information and referrals to diverse resources.
- Maintains case record documents providing thorough documentation in a timely
- Maintains partnerships with community-based organizations (local schools/ school districts or health and community non-profits or stakeholders in the CHWs community/neighborhood.
- Delivers health education classes.
- Assists other CHWs in helping families create goals and care plans of action to engage healthy behaviors.
- Identifies patient compliance barriers, still ensuring quality of care coordination delivery.
- Prepares reports of tasks and activities and keeps complete records of outreach events, activities and contacts along with people reached.
- Provides updated inventory of health education materials and community resources in the area.
- Performs other job-related duties as required.
- High School Diploma- no experience necessary and Community Health Worker Certification must be obtained within at least 12 months of hire.
- High School Diploma and 1 year of experience, and Community Health Worker Certification obtained within at least the last 12 months.
- High School Diploma and 3 years of experience, and Community Health Worker Certification obtained within at least the last 12 months.
