What are the responsibilities and job description for the Patient Navigator 2 position at University of California San Francisco?
Please note: This is a temporary position.
Reporting to the Practice Supervisor, the Dermatology Patient Navigator will provide individualized assistance, support and coordination to patients in multiple Dermatology Specialty Practices. They will serve as a liaison between the referring provider, the patient and services they may require throughout the practice. The Navigator provides patient care coordination by serving as a communication bridge, when needed, between the patient and the practitioners and services they may require during their care.
The Navigator is a team player who works closely with others and who is flexible and dependable in dealing with the changing priorities. They are a self-reliant individual who analyzes their knowledge of practice operations in order to problem-solve, prioritize and facilitate complex transactions. They will be required to adapt to new workflows and policies as it relates to the growth of Dermatology Specialty Practices.

Required Qualifications
- Familiar with APeX/Epic Electronic Medical Record.
- Ability to communicate effectively, both over the telephone and directly in person.
- Basic keyboard and computer skills (Microsoft Office) and basic math skills.
- Associates degree and 6 months related experience; or equivalent combination of education and experience.
Preferred Qualifications
- Bi-lingual or multi-lingual capability (Spanish, Cantonese)
- Experience with insurance verification (plans), patient financial services, medical record data abstraction, or data analysis
- Experience with Epic electronic medical record.
- Dermatology experience
