Job Summary

Registered Nurses (RN) in the Home Care Services department at UC Davis Health provide care to adult and pediatric patients with a variety of health care needs including post-operative, medical, palliative, and end-of-life care. Patient conditions include both acute and chronic conditions spanning across many care specialty areas and age ranges.

The Home Care Services Department is comprised of the Home Health, Hospice, and Palliative Care programs. These programs offer patients the opportunity to receive skilled nursing care in the comfort of their home. Home care nurses work collaboratively in an inter-professional team environment to provide the highest level of patient-centered care focused on supporting patient goals.

The RN identifies and implements nursing interventions and evaluates results for a wide variety of patient populations, directs and acts as resource to other members of the nursing team as assigned, and promotes compliance and support for policies. The primary responsibilities for this position consist of providing total, comprehensive, continuous, and patient-centered care that includes assessing, documenting, planning, implementing, teaching, developing, and evaluating a comprehensive nursing plan of care; delivering direct patient care; ensuring patient satisfaction; training and delegating duties; and adhering to university policies and procedures as well as federal, state, and local regulations.

Autonomous nursing care is the ability of a nurse to assess and provide nursing actions as appropriate for patient care populations based on competence, professional expertise, and knowledge. The nurse is expected to practice autonomously consistent with ANA Standards of Practice and Professional Performance. Independent judgment is expected to be exercised within the context of interdisciplinary and multidisciplinary approaches to patient care.

Provides direct patient care to a diverse patient population. Applies knowledge, experience, and judgment to determine the importance of a situation, set priorities, and use abstract thought to evaluate clinical situations. Participates in patient and staff education. Demonstrates leadership skills. Demonstrates accountability for own practices as defined by the Nurse Practice Act.

Minimum Qualifications - For full consideration, applicants are encouraged to upload license and/or certification if required of the position

  • Valid California Registered Nurse License without restriction
  • American Heart Association (AHA) or American Red Cross Basic Life Support (BLS), or UC Davis Health CPR
  • Required to hold a valid California driver's license, have a driving record that is in accordance with local policies/procedures, and/or enroll in the California Employer Pull Notice Program
  • Verification of graduation from an accredited school of nursing.
  • One (1) year inpatient experience as a bedside RN on an adult patient care unit with a variety of healthcare needs including acute and chronic conditions across many specialty areas.
  • One (1) year current home care experience with Oasis experience, or evidence of education in home health, hospice, or palliative care.
  • Ability to read, write, speak, and understand the English language.
  • Excellent written and verbal communications skills.
  • Ability to utilize medical terminology appropriately.
  • Knowledge of current trends in healthcare and interdisciplinary clinical practice.
  • Ability to remain calm under pressure.
  • Effective interpersonal skills.
  • Demonstrates accountability and professionalism.
  • Adapts to change in a positive, professional, and productive manner.
  • Knowledge of performance improvement and evidence-based practice.

  • Ability to assess, plan, implement, and evaluate patient care, taking into consideration protective interventions, development (behavioral and physical), and physiological characteristics specific to the patients.

Preferred Qualifications

  • Certified Oasis Specialist Clinical (COS-C)
  • Certified in Hospice and Palliative Nursing (CHPN)
  • Ambulatory Case Management
  • Medical/Surgical Certification
  • BSN preferred.
  • Two (2) years experience as a home health case manager in a Medicare-certified home health agency.
  • Two (2) years inpatient experience as a bedside RN on a nursing unit.

The UC Davis Home Care Services Home Health program is a full-service agency offering the best in home health care. Our Home Health program is nationally ranked among the highest in patient satisfaction. Home Care Services is made up of three primary programs: Hospice (inpatient and home care), Home Health, and Home-Based Palliative Care. These programs offer patients the opportunity to received skilled nurse care in the comfort of their home. Home care nurses work collaboratively in an inter-professional team environment to provide the highest level of patient-centered care focused on supporting patient goals. This position would be a part of the Home Health Program.

Department Specific Job Scope

The Home Health RN Case Manager is responsible for direct patient care and associated activities of adult home health patients.

The incumbent primarily works in the field as this is an in-home care position. Occasionally, the incumbent works in the HCS departmental office located in Rancho Cordova for meetings, training, and additional, administrative tasks.

Work Environment
