Role Description This is a full-time, on-site Pharmacist role located in San Antonio, TX. The Pharmacist will review, verify, and dispense medications, ensuring safe, accurate, and timely delivery of therapies to inpatients and outpatients in accordance with evidence-based guidelines and regulatory standards. Responsibilities include collaborating with physicians, nurses, and other healthcare professionals on medication selection, dosing, and monitoring, as well as providing medication counseling and education to patients, families, and caregivers. The Pharmacist will conduct clinical reviews such as medication reconciliation, therapeutic drug monitoring, and renal/hepatic dose adjustments, while identifying and resolving actual or potential drug therapy problems. The role also involves maintaining accurate records, participating in quality improvement and antimicrobial stewardship or other clinical initiatives, supervising and guiding pharmacy technicians and students, and supporting the academic and teaching mission of the health system.
Qualifications
  • Licensure and education: Doctor of Pharmacy (PharmD) or equivalent from an ACPE-accredited school and current or eligibility for Texas pharmacist licensure.
  • Clinical pharmacy expertise: Strong knowledge of pharmacotherapy, disease state management, and evidence-based guidelines, with experience in hospital or health-system pharmacy practice preferred.
  • Medication safety and regulatory knowledge: Familiarity with medication safety principles, formulary management, sterile and non-sterile compounding standards, and applicable regulations (e.g., Joint Commission, USP, state and federal pharmacy law).
  • Patient care and communication skills: Ability to provide clear, compassionate medication counseling, engage in shared decision-making, and communicate effectively with diverse patients and interdisciplinary teams.
  • Collaborative and teaching skills: Experience working in multidisciplinary care teams and interest in precepting students, residents, or supporting an academic environment.
  • Analytical and technology skills: Proficiency with electronic health records, pharmacy information systems, and clinical decision support tools, with strong attention to detail and problem-solving abilities.
