Key Responsibilities

• Provide comprehensive primary care, including assessment, diagnosis, treatment, and follow-up.
• Manage acute and chronic conditions, including diabetes, hypertension, cardiovascular disease, respiratory conditions, and other common primary care needs.
• Perform preventive health visits, screenings, immunizations, and patient education.
• Order and interpret laboratory, imaging, and other diagnostic studies.
• Prescribe medications and therapeutic interventions within scope of practice.
• Coordinate care with specialists, behavioral health providers, hospitals, and community resources.
• Maintain accurate and timely documentation in the electronic health record.
• Participate in quality improvement, population health, and clinical performance initiatives.
• Collaborate with the interdisciplinary care team to address medical and social factors affecting patient health.
• Support the health center's commitment to accessible, equitable, culturally responsive care.

Qualifications

Required:

• Graduate of an accredited Nurse Practitioner program.
• Current and unrestricted Nurse Practitioner license in New York State.
• Current national board certification appropriate to the patient population.
• Current BLS certification.
• DEA registration or eligibility, as applicable.
• Minimum 3 years of independent or advanced primary care clinical experience.
• Strong clinical judgment and demonstrated ability to manage medically and socially complex patients.
• Excellent communication, documentation, and interdisciplinary collaboration skills.

Preferred:

• Previous experience in an FQHC, community health center, rural health center, or other safety-net setting.
• Experience caring for underserved or medically complex populations.
• Familiarity with population health, UDS measures, PCMH, value-based care, and quality improvement.
