Key Responsibilities
- Provide comprehensive primary care, including assessment, diagnosis, treatment, and follow-up.
- Manage acute and chronic conditions, including diabetes, hypertension, cardiovascular disease, respiratory conditions, and other common primary care needs.
- Perform preventive health visits, screenings, immunizations, and patient education.
- Order and interpret laboratory, imaging, and other diagnostic studies.
- Prescribe medications and therapeutic interventions within scope of practice.
- Coordinate care with specialists, behavioral health providers, hospitals, and community resources.
- Maintain accurate and timely documentation in the electronic health record.
- Participate in quality improvement, population health, and clinical performance initiatives.
- Collaborate with the interdisciplinary care team to address medical and social factors affecting patient health.
- Support the health center's commitment to accessible, equitable, culturally responsive care.
Qualifications
Required:
- Graduate of an accredited Nurse Practitioner program.
- Current and unrestricted Nurse Practitioner license in New York State.
- Current national board certification appropriate to the patient population.
- Current BLS certification.
- DEA registration or eligibility, as applicable.
- Minimum 3 years of independent or advanced primary care clinical experience.
- Strong clinical judgment and demonstrated ability to manage medically and socially complex patients.
- Excellent communication, documentation, and interdisciplinary collaboration skills.
Preferred:
- Previous experience in an FQHC, community health center, rural health center, or other safety-net setting.
- Experience caring for underserved or medically complex populations.
- Familiarity with population health, UDS measures, PCMH, value-based care, and quality improvement.
