Job Summary
We are seeking a compassionate and dedicated Registered Nurse for our Home Health division. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being while promoting independence. This role requires strong clinical skills, excellent communication abilities, and a passion for patient-centered care.
Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans.
- Administer medications and treatments as prescribed, including IV infusions and injections.
- Monitor patient vital signs and overall health status, documenting changes accurately in electronic health records (EHR).
- Provide education to patients and families regarding health management, medication administration, and home care techniques.
- Collaborate with interdisciplinary teams to coordinate patient care and ensure adherence to treatment plans.
- Assist with activities of daily living (ADLs) such as bathing, dressing, and mobility support.
- Perform wound care, catheterization, tube feeding, and other nursing procedures as required.
- Maintain compliance with HIPAA regulations and ensure the confidentiality of patient information.
- Participate in ongoing training and professional development to enhance nursing skills.
Skills
The successful candidate will possess a range of skills including but not limited to:
- Proficiency in EMR systems such as Epic or Cerner.
- Experience in critical care settings including ICU or Level I/II trauma centers.
- Knowledge of medical terminology, anatomy, physiology, and infection control practices.
- Familiarity with ICD-9/ICD-10 coding for accurate medical documentation.
- Competence in performing phlebotomy, venipuncture, and specimen collection/processing.
- Ability to manage acute pain effectively and provide hospice & palliative medicine care.
- Experience working with diverse populations including geriatrics, pediatrics, and individuals with developmental disabilities.
- Strong assessment skills for triage and diagnostic evaluation.
- Knowledge of case management principles and discharge planning processes.
- Understanding of health coaching techniques to promote wellness among patients. Candidates with additional experience in areas such as dialysis, trauma medicine, behavioral health, or home & community care are highly encouraged to apply.
Pay: $45.00 - $75.00 per hour
