Job Summary
We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our healthcare team. The ideal candidate will possess a strong commitment to providing high-quality patient care in various settings, including hospitals, long-term care facilities, and outpatient clinics. This role requires excellent clinical skills, effective communication abilities, and a passion for supporting patients and their families.
Duties
- Provide direct patient care under the supervision of registered nurses and physicians.
- Administer medications and treatments as prescribed, ensuring compliance with safety protocols.
- Conduct patient assessments, including vital signs monitoring and documentation of medical history.
- Assist in developing and implementing individualized care plans for patients.
- Collaborate with interdisciplinary teams to coordinate patient care and ensure optimal outcomes.
- Educate patients and families on health management, medication administration, and post-discharge instructions.
- Maintain accurate medical records and documentation in compliance with HIPAA regulations.
- Participate in infection control measures and ensure a safe environment for patients.
- Support patients with activities of daily living (ADLs) as needed.
Skills
Candidates should possess a variety of skills to excel in this role, including but not limited to:
- Proficiency in EMR/EHR systems such as kinser.
- Experience in critical care settings (ICU, PICU) and emergency medicine.
- Knowledge of medical terminology, ICD-9/ICD-10 coding, and medication administration.
- Familiarity with long-term care practices, including Alzheimer’s care, dementia care, hospice care, and geriatrics.
- Ability to perform IV infusion, venipuncture, catheterization, basic life support, blood draw, foley catheter management
- Strong patient assessment skills with experience in acute pain management and physical examinations.
- Experience working with diverse populations including pediatrics and individuals with developmental disabilities.
- Excellent communication skills for effective interaction with patients, families, and healthcare teams.
