Job Overview
We are seeking compassionate and dedicated Home Health Aides to provide essential care and support to individuals in their homes. Duties
- Assist clients with Activities of Daily Living (ADLs) including bathing, dressing, grooming, and toileting.
- Provide companionship and emotional support to enhance the well-being of clients.
- Monitor and document vital signs, changes in patient conditions, and report any concerns to healthcare professionals.
- Administer medications as prescribed and ensure proper medication management.
- Assist with mobility using equipment such as Hoyer lifts when necessary.
- Provide specialized care for individuals with Alzheimer’s disease, dementia, or other cognitive impairments.
- Support clients with cleaning tasks, meal preparation, laundry, and other household duties as needed.
- Maintain accurate medical documentation in compliance with HIPAA regulations.
- Collaborate with healthcare teams to develop individualized care plans that meet client needs.
Skills
- Strong patient observation skills to identify changes in health status.
- Experience in assisted living or nursing home settings is preferred.
- Familiarity with EMR/EHR systems for efficient medical documentation.
- Knowledge of medical terminology and procedures relevant to home health care.
- CPR certification is a plus; training may be provided if necessary.
- Experience in catheter care and hospice care is advantageous.
- Ability to work effectively with individuals who have developmental disabilities or require memory care support.
- Proficiency in caregiving tasks including cooking, cleaning, and laundry for client comfort.
