What are the responsibilities and job description for the UR Coordinator/ RN Case Manager Full Time- Home Health and Hospice position at Universal Health Services, Inc.?
Reporting to the Director of Home Health/Hospice, the quality improvement/case manager nurse will be responsible for providing safe, appropriate nursing care to patients in their place of residence and will work with the Director to implement the Home Health Quality Improvement Program.
QualificationsMINIMUM ESSENTIAL EXPERIENCE
Minimum (1) year prior professional nursing experience required
Home health experience preferred. Knowledge of Medicare and Title 22 regulations/guidelines
REQUIRED LICENSURE
Current California license as a Registered Nurse required
Current BCLS (CPR) required
Current California driver's license and vehicle insurance required
