What are the responsibilities and job description for the PROGRAM COORD - IOP/PHP position at Universal Health Services, Inc.?
The ClinicalCare Coordinator, under the direction of the program manager, is an active member of the patient treatment team.
- Provide milieu management and continuous patient caresupervision, interactions and role modeling to patients.
- Provide support at the front desk,including:
- Maintaining patient transportation schedule, including scheduling Lyfts as needed
- Answering phones
- Completing phone assessments with potential clients and adding them to Windmoor Call Log
- Providing required paperwork for intakes, discharges, and weekly outcome measures
- Entering outcome measures in Reflections tracking database
- Supervising and updating daily attendance tracker
- Tracking patient attendance, absences, and length of treatment
- Following no-call/no-show protocol as needed to ensure patient safety
- Meeting with potential stepdown patients from inpatient units for registration
- Providing support to clinicalstaff by completing admission assessments, treatment plans, and/or facilitating group sessions.
- Assisting patients with obtaining resources and referrals as needed.
- Participating in additional tasks as needed through assignment by the program manager.
Qualifications
Graduate degree in social work, mental health counseling, marriage and family therapy, or related field.
A minimum of 1 year experience working with individuals with persistent mental illness (PMI) and/or substance abuse.
CPR certification and successful completion of restraining procedures training within 30 days of employment.
