The Discharge Coordinator will work with parents, outpatient providers, and therapists to establish d\safe and supportive aftercare plans.

Education/Experience: Bachelor's Degree required. 1 year experience in Case Management preferred. Experience and/or knowledge of mental health systems.

Skills: Organizational skills, computer skills including excel, strong communication skills.
