Quail Run Hospital is a 102-bed, acute care psychiatric hospital located in Phoenix, Arizona. Quail Run features individual units for adolescents, adults, and seniors, and offers inpatient acute care, partial hospitalization, and intensive outpatient programs. On average, over 10,000 patients receive care from our compassionate health care team each year at Quail Run Behavioral Health .

Summary:
The Discharge Planner assumes responsibility for the overall resource needs of the patient.. This includes discharge planning, placement of the patient, aftercare needs, advocacy needs, housing referrals, and entitlement needs.
Qualifications
Job Requirements
- Bachelor’s degree from an accredited college or university in Social Work or related mental health field required.
- A minimum of two (2) years direct clinical experience in a psychiatric or mental health setting and direct experience in patient/family assessment required.
- Experience in working with programs and outside referrals preferred
- Level 1 Arizona Fingerprint Clearance card or be able to obtain within 30 days of hire.
