Responsibilities
- Serve as a primary clinical resource for cardiology patients, providing assessment, triage, and care coordination.
- Evaluate patient symptoms and concerns via phone, patient portal, and other communication channels to determine appropriate next steps.
- Collaborate closely with cardiologists, advanced practice providers, and the care team to ensure timely, high-quality patient care.
- Coordinate diagnostic testing, procedures, referrals, and follow-up care for patients with cardiovascular conditions.
- Educate patients and families on cardiac diagnoses, medications, treatment plans, and heart-healthy lifestyle modifications.
- Review laboratory, imaging, and diagnostic test results and communicate provider recommendations to patients.
- Support patients managing chronic cardiovascular conditions such as heart failure, hypertension, arrhythmias, and coronary artery disease.
- Document patient interactions, assessments, and care plans accurately within the electronic health record (EHR).
- Prioritize and manage multiple patient needs in a fast-paced specialty clinic while delivering exceptional patient-centered care.
- Current Registered Nurse (RN) license in the State of Iowa.
- Associate Degree in Nursing (ADN) required;
- Previous cardiology, specialty clinic, ambulatory care, critical care, telemetry, or medical-surgical nursing experience is great, but not required.
- Strong assessment, critical thinking, and clinical decision-making skills.
- Excellent communication and patient education abilities.
- Ability to prioritize multiple responsibilities and adapt in a fast-paced clinical environment.
- Experience with Epic a plus!
