Responsibilities
- Conduct comprehensive assessments and develop individualized care plans
- Deliver skilled nursing care in accordance with physician orders and organizational standards
- Monitor patient status, evaluate responses to treatment, and document all care accurately
- Educate patients and families on disease management, medications, and home safety
- Collaborate with interdisciplinary team members to ensure continuity of care
Education:
- Must have an Associates Degree in Nursing. (Must obtain BSN within 12 years of hire date)
- Active Iowa Registered Nurse licensure issued by the Iowa Board of Nursing
- Within 1 year of hire, need to obtain RN licensure in the State of Wisconsin
- Must have a valid driver's license and proof of automobile insurance coverage
