Responsibilities:Key Responsibilities
- Coordinate comprehensive care for patients with chronic respiratory illnesses such as COPD, asthma, interstitial lung disease, and sleep disorders.
- Perform clinical assessments and develop individualized care plans in collaboration with pulmonologists and advanced practice providers.
- Facilitate communication among the care team, patients, and families to ensure continuity of care across inpatient, outpatient, and community settings.
- Manage and track follow-up care, diagnostic testing, medication refills, and treatment adherence.
- Provide education and counseling on disease management, medication use, smoking cessation, oxygen therapy, and lifestyle modifications.
- Support transitions of care following hospitalizations or emergency visits, ensuring timely follow-up and preventing readmissions.
- Assist with prior authorizations, insurance coordination, and documentation of clinical encounters in the electronic health record (EHR).
- Monitor quality metrics and participate in quality improvement initiatives related to pulmonary health outcomes.
- Serve as a resource and advocate for patients navigating complex care systems.
- Current, active Registered Nurse (RN) license in the State of Iowa
- Graduate of an accredited nursing program (ADN or BSN)
- Basic Life Support (BLS) certification
- Strong organizational, communication, and critical-thinking skills
- Bachelor of Science in Nursing (BSN) - preferred
- Minimum 2 years of clinical experience in pulmonology, internal medicine, or chronic disease management - preferred
- Case management or care coordination experience - preferred
- Familiarity with Epic or other electronic health record systems - preferred
- Certified Case Manager (CCM) or Care Coordination certification (or willingness to obtain) - preferred
