Responsibilities
Under general supervision, collects, evaluates and prepares performance improvement data and/or reports and/or other complex statistical data. Prepares statistical and narrative reports and recommendations and participates in the analysis and interpretation of data as appropriate. Develops, maintains, and ensures quality control of databases and assists in development and implementation of measurement systems. Provides full range of administrative and meeting support. Will independently administer assigned responsibilities, ensuring excellent Customer Service.
Quality Assurance & Process Improvement
- Collect, analyze and present data for administrative, clinical decision making, and regulatory requirements. Prepare and present reports for appropriate facility, system and other external agencies. Provide oversight to projects including data abstracting and entry as appropriate. Develop and/or utilize databases and software as required for collection, tracking and analysis of data. Examples include:
- HPMS
- DataPACE3
- HOS-M
- With the assistance and guidance of the PACE QAPI Manager:
- Prepares Performance Improvement committee agenda and materials.
- Takes minutes at Performance Improvement and Participant Advisory committee meetings.
- Updates QAPI Dashboard/Work Plan on regular basis
- Maintains Grievance Log and Appeals Log
- Track, trend, and document risk reports
- Prepares reports for submission to regulatory and national benchmarking organizations.
- Facilitates and participates in required auditing.
- Assists with Annual Program Evaluation and QAPI Work Plan Development/Implementation.
- Prepares and distributes various Participant Appeals and Grievance information to various parties. Audits and reports on compliance with processes.
- Assists QAPI manager and Directors with preparation of materials and packets for internal and external appeal reviews
- Utilizes specified software to prepare data, as appropriate to the individual position (example RL Feedback and Risk reporting).
- Assists Interdisciplinary team with interpretation and implementation of PACE policy/procedures and regulatory requirements.
- Assists with Performance Improvement Team facilitation and maintenance of documentation.
- Assists with Compliance Audit Processes and monitoring of staff compliance issues.
- Level II’s
- Identifies reporting severity level and completes required reporting process
- Conducts Root cause Analysis in coordination with Medical Director.
- Track, trend, log and document Level II’s
- Assist PACE Administration with Policy and Procedure review, revision and document maintenance.
- Assist all staff with QAPI education and processes as needed.
