Job Summary
The Registered Nurse (RN) provides skilled nursing care to patients in their homes. This role includes planning, organizing, and directing home care services while promoting patient and family independence. The RN uses community resources and clinical expertise to deliver high‑quality, compassionate care.
Essential Job Responsibilities
Patient Care
- Complete initial patient and family assessments to determine home care needs.
- Perform comprehensive physical assessments and review medical history.
- Reevaluate patient needs on a regular basis.
- Initiate and update individualized plans of care based on patient status.
- Use assessment data to establish nursing diagnoses.
- Develop care plans with measurable goals and involve the patient and family in the process.
- Provide preventive, therapeutic, and rehabilitative nursing procedures.
- Administer medications and treatments as prescribed.
- Offer direct patient care in accordance with the State Nurse Practice Act.
- Educate and counsel patients and families regarding health needs.
- Provide health care instructions based on the plan of care.
- Identify and implement discharge planning needs.
- Serve as Case Manager when assigned, coordinating all aspects of patient care.
Communication
- Prepare accurate and timely clinical notes.
- Update the primary physician as needed and at least every 60 days.
- Communicate changes in patient condition and obtain physician orders as required.
- Coordinate care with community health professionals and service providers.
Additional Duties
- Participate in on‑call rotation per agency policy.
- Ensure necessary equipment, supplies, and services are arranged for patient care.
- Instruct, supervise, and evaluate home health aide performance every two weeks.
Job Types: Full-time, Part-time, PRN
Pay: $36.79 - $44.30 per hour
