Unity House is dedicated to improving the lives of individuals in our community, and our Supportive Housing Program is seeking a compassionate and skilled Mental Health RN to join our team.
This position provides integrated health management and nursing support services to individuals living in supportive housing programs who are experiencing Serious Mental Illness (SMI) and/or have been impacted by Domestic Violence (DV).
The Mental Health RN plays a vital role in promoting physical and behavioral health wellness, supporting individuals in managing chronic health conditions, and helping participants navigate and access community-based healthcare resources.

Key Responsibilities
- Provide integrated health care and care management services to individuals living with Serious Mental Illness and/or impacted by Domestic Violence.
- Work as part of a multidisciplinary team serving participants in supportive housing programs.
- Conduct basic health assessments and identify unmet medical needs.
- Support participants in understanding and managing their health conditions and treatment recommendations.
- Assist with person-centered care planning.
- Provide referrals and coordination with community medical providers and specialists.
- Deliver treatment, rehabilitation, and supportive services within the RN scope of practice.
- Promote wellness, self-management, preventive care, and healthy lifestyle practices.
- Build and maintain strong relationships with community partners and healthcare providers.
- Complete required documentation and administrative tasks accurately and on time.
- Foster a culture of inclusion, belonging, and trauma-informed care.
Qualifications
- Current, unrestricted NYS Registered Nurse (RN) License.
- Minimum of 1 year of experience working in mental health, substance use, supportive housing, or community-based settings.
- Experience working with individuals living with Serious Mental Illness (SMI) and/or survivors of Domestic Violence preferred.
- Valid NYS driver’s license and reliable vehicle.
- Ability to work independently and collaboratively as part of a multidisciplinary team.
- Proficiency with computers and electronic documentation.

Salary : $72,917 - $75,866
