Unity House of Troy is seeking a proactive, compassionate, and dedicated Housing Case Manager to join our Domestic and Sexual Violence Program. This role is ideal for someone committed to supporting survivors through trauma-informed, client-centered care while helping them achieve safety and housing stability.
Position Summary
The Housing Case Manager provides advocacy and comprehensive case management services to individuals impacted by domestic violence. This position plays a critical role in supporting survivors through safety planning, housing navigation, and ongoing emotional and practical support.
Key Responsibilities
- Provide victim-centered, trauma-informed case management services
- Conduct safety planning, needs assessments, and individualized service planning
- Advocate on behalf of clients to access housing and community resources
- Maintain regular, supportive contact with clients
- Assist clients in navigating housing options, applications, and requirements
- Provide referrals and coordinate with community partners
- Complete timely and accurate documentation, reports, and compliance activities
- Promote a culture of inclusion, respect, and belonging
- Represent and advocate for the program and the individuals we serve
Qualifications
- Bachelor’s degree in Human Services or related field (preferred) with 1 year of experience
- Associate’s degree with 2 years of relevant experience (preferred)
- High School Diploma/GED with 3 years of relevant experience may be considered
- Experience working with survivors of domestic violence or similar populations strongly preferred
- Knowledge of housing systems and community resources is a plus
- Valid, clean New York State Driver’s License and reliable transportation required
