JOB DESCRIPTION
PRIMARY REGISTERED NURSE
Position Summary
The primary Registered Nurse (RN) is responsible for the overall care of the patients and families. The primary RN directs the activities of other nursing personnel and works in conjunction with interdisciplinary team members.

Qualifications
- Current unrestricted Registered Nurse licensure from the State of Wisconsin
- Minimum of 2 years RN experience required, equivalent nursing experience may be considered
- Valid Wisconsin Driver’s License, reliable transportation and proof of current automobile insurance coverage

Key Job Duties
- Provide clinical and supporting case management based on patient’s diagnosis and end-of-life symptoms.
- Complete initial, comprehensive, and updated assessments of patient and family needs.
- Develop, implement and review care plans for patient and family. Update care plan as the needs of the patient and family change
- Prepare for and participates in interdisciplinary team meetings as scheduled
- Communicate with all interdisciplinary team members
- Report appropriate findings and communicate significant changes to attending physician and processes physician/NP orders
- Teach family members patient care skills and encourage their participation including, but not limited to, pain control, medication management, dressings, hygiene and nutrition
- Administer and titrate medication according to physician/nurse practitioner orders
- Delegate nursing duties to LPN and supervise care provided by CNA
- Prioritize daily work and make adjustments taking into account the needs of patients, Unity and other staff
- Complete new patient admissions as needed
- Maintain level of competency in required areas (primary and admissions roles)
- Comply with all applicable local, state and federal laws and regulations as well as policies and procedures of Unity
- Satisfy mandatory training requirements regarding local, state and federal regulations and/or Unity’s policies and procedures
- Consistently and concisely document activity according to department protocols and standards
- Promote and maintain confidentiality
- Maintain personal and professional boundaries
- Perform other job-related duties as assigned
- Maintain regular and reliable attendance
- Shares in providing 24-hour, seven day-a-week coverage

Physical and Environmental Requirements
Essential Physical/Environmental/Technical Skills:
Employees must have the ability to; operate computers, telephones, and other office equipment, communicate both verbally and in writing, sit for prolonged periods of time as required by position, walk, bend and lift up to 35 pounds on occasion, organize and prioritize tasks.
Exposure to Hazards:
In the course of duties there may be a risk of “occupational exposure”. An occupational exposure defined by OSHA is “reasonable anticipated skin, eye, mucous membrane, nonimpact skin, or parental contact with blood and other potentially infectious material that may result from the performance of an employee’s duties.” This may also include other potentially hazardous materials.
Personal Protective Equipment (PPE):
PPE is one measure available to all employees to help reduce the risk of contracting blood borne illnesses and other diseases. It is the employee’s duty to assess the need for PPE’s and the appropriate kind to be used. Furthermore, it is the employee’s responsibility to wear and use the PPE that has been made available.

Salary : $65,700 - $83,200
