At Unity Hospice, we bring peace of mind and comfort to patients and families when it matters most. We’re growing and looking for a PRN Referral & Intake Nurse (LPN) to join our team and serve as a key connection point for patients, families, and referral partners across all Unity territories.
This role is ideal for someone who thrives in a fast-paced environment, enjoys coordination and communication, and is passionate about supporting patients at the very start of their hospice journey.
PRN Requirements: Must work a minimum of 16 hours per month, at least one holiday per year, and weekends as determined by the leader/designee based on departmental need. Typical hours are 8:00 a.m. – 5:00 p.m.
This role provides coverage across all Unity territories.

What You’ll Do

- Managing incoming referrals and gathering patient medical information
- Communicating with hospitals, clinics, and care teams
- Coordinating initial visits with patients, families, and admissions staff
- Verifying insurance coverage and obtaining necessary orders
- Supporting eligibility review and following up on pending referrals
- Maintaining accurate and timely documentation
Requirements
- Current unrestricted Licensed Practical Nurse licensure from the State of Wisconsin
- Valid WI Drivers’ License, reliable transportation, and proof of current automobile insurance coverage
